Semen analysis assesses several parameters, and a result being outside the reference range is not the same as being infertile. The parameters measured are volume, sperm concentration, total sperm number, motility (particularly progressive motility), and morphology. Two points about interpretation that cause a great deal of unnecessary alarm. First, the reference values are derived from men who achieved a pregnancy within twelve months — they are population reference points, not thresholds separating fertile from infertile. Men below them conceive naturally, and men above them sometimes do not. Second, results vary substantially between samples from the same man, so a single abnormal analysis should always be repeated, typically after several weeks. Collection conditions affect results: abstinence of two to seven days, complete collection including the first portion, and prompt delivery to the laboratory all matter. Morphology in particular is frequently over-interpreted — a low percentage of normal forms is common and does not on its own indicate a serious problem. Have the full report interpreted by a urologist or andrologist rather than reacting to any individual number.
