Can PGT-SR prevent miscarriage?
It can reduce the chance of a miscarriage caused by an unbalanced chromosome result, by selecting embryos with a balanced complement before transfer. That is a genuine benefit. What it has not been shown to do is produce more babies overall than trying naturally. ASRM (2026) states that the efficiency of PGT-SR versus expectant management has not been established, owing to the lack of prospective trials comparing subsequent pregnancy outcomes. The comparison to weigh: Natural conception: live birth rates as high as 70 to 71% reported in observational studies of translocation carriers, with miscarriage rates of 29 to 30%. No IVF, no cost, but the emotional burden of possible further losses. PGT-SR: fewer miscarriages along the way, but requiring IVF, with its own success rates. One study of PGT-SR cycles reported a live birth rate of 56% per euploid transfer and 38% per started cycle. Note the difference between those two figures โ per started cycle is what you should compare against natural conception, because it includes cycles that produce no transferable embryo. A balanced translocation substantially reduces usable embryos โ on average at least 4.5 blastocysts were needed for a good chance of one transferable embryo in one study, and euploidy rates of 17 to 22% depending on maternal age have been reported. For couples who have had multiple losses, avoiding further miscarriages may matter more than the overall statistics. That is a legitimate reason to choose PGT-SR, and it should be presented as such rather than as a superior success rate.
